Example case: Client called when mother was hospitalized in ICU and uncertain about rehab options. Result: Call, In-person visit, family meeting with medical team, discussion of care plan for safely transitioning home with appropriate equipment, support and resources, completed Advanced Healthcare Directive, and Follow-Up call.
